Test Reservation

Complete this form and send it with your check for $35 to the address on the
form. Don't delay, sessions are limited to 15 dogs, and they fill up quickly.
We will call you with the location when we receive your reservation.

Certificate of Health
Your vet must complete this before we can test your dog. You must provide
this document before or at the practice session or your dog cannot
participate!
